Complete and submit the Parent Consent Form with your child’s artwork.  Your child MUST have your signature (or electronic signature) on a Parent Consent Form in order to participate. Fill in all required fields. If you do not have information for a required field type in "N/A".

Project S.N.A.P. reserves the right to select and use the student’s submission in any mural created for the Project S.N.A.P Mosaic Mural Project. In addition, Project S.N.A.P has the right to change an image size or crop it, in order to make it suitable for use by Project S.N.A.P.. The student grants to Project S.N.A.P the right to use the student’s submission, without limitation, for any purposes (including commercial applications) that would benefit Project S.N.A.P.
